In December, a truly special birth took place at Nadezhda Hospital. Alexandra and Victoria took their first breath here—twin girls with a shared placenta, born naturally.
This type of birth is extremely rare and comes with challenges, risks, and critical decisions. However, the medical team and the mother managed the situation in the best possible way.
The twins were born at 37 weeks of gestation, in the hands of Dr. Krasina Licheva, midwife Boryana Chukanova, Dr. Kristin Yaneva, and neonatologist Dr. Detelina Stoichkova.
Dr. Krasina Licheva: An Inspiring Birth with Carefully Measured Risk
“My first meeting with Desi regarding this pregnancy was at 6 weeks of gestation, when the pregnancy was confirmed. I should mention that Desi is an extremely petite woman. I could hardly believe it when I saw two beating embryos. She was no less surprised than I was.
A few weeks later, it was confirmed that there was only one placenta. A twin pregnancy is already considered high-risk, and one with a single placenta (monochorionic) can lead to twin-to-twin transfusion syndrome, which increases the risk for both fetuses. This requires strict monitoring and rapid intervention when necessary. Fortunately, Desi reached 36+ weeks of gestation.
And then the day arrived. Desi had regular uterine contractions. After a detailed vaginal and ultrasound examination of the babies’ positions and sizes, we made a mutual decision to attempt a natural birth.
A vaginal delivery of twins with one placenta is exceptionally rare due to the complications that may arise. The team was prepared to change the plan at any moment. Desi was thoroughly informed about what to expect.
This birth was a carefully measured risk that spared the mother abdominal surgery and brought the benefits of natural birth for the babies.
It was an emotional moment for the parents and the entire team, supported by the mother’s explicit wish and our professionalism.
With the newborn twin girls, the women in the family become three. May they be healthy and happy—and the father now has his hands full! I congratulate Desi on her courage. May she be an example to future mothers not to fear natural childbirth.”
